29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: On or about October 11, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours.
