29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: In the Rolls Label Department, employee engaged in material handling activities were exposed to struck-by hazards in that refresher training in relevant topics was not provided to the forklift operator when the operator was involved in accident or near-miss incident. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
