29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A) AK Tube, worksite located 30400 E. Broadway, Walbridge, Ohio, 43567: On or about January 4, 2021, the employer did not assure employees were protected from entering the point of operation while using Enerpac Hydraulic Press that resulted in an amputation of the tip of the left index finger.
