29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: (a) Cranston facility: 2020 OSHA Form 300, Instance 3, Case # 174554: An employee was hospitalized and tested positive for COVID-19 on 11/21/2020 and passed away on 12/14/2020. The employer determined that the employee's illness was work-related on or about 11/23/2020 and did not complete the OSHA 301 Incident Report, or equivalent, until 1/05/2021, which is more than seven (7) calendar days.
