29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about December 11, 2020, during the removal/replacement of roofing material/sealer at a job site located at 780 Schocalog Rd, Copley, Ohio 44321; the employer did not ensure employees were protected from falling through holes (skylights) in the working surface. The employees were not protected by personal fall arrest systems, covers, or guardrail systems erected around such holes exposing the employees to a fall hazard of approximately 16.3 feet to the surface below.
