Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (c)(4)(ii)(B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; On or about 11/30/2020 the employer did not provide adequate machine specific lock out procedures that provided means to secure all energy sources prior to loaders and operators conducting tasks on the AIDA Straightener Feeder (model # LFG-600) such as setup and adjustments in that the procedures utilized were not lockout procedures that covered the equipment. They were procedures that covered the interlocked gate that employees had to enter to access the Feeder. The gate had safety procedures attached to them titled "Safety Plug Procedures". By following the "Safety Plug Procedures" instead of lockout procedures, the pneumatic power to the coil guides and the coil positioning handle were not addressed resulting in employees being exposed to a struck by hazards.
