29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a. Muncie Power Products, Inc. located at 980 Claycraft Road, Suite B, Gahanna, OH 43230: On or about November 26, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on December 3, 2020.
