29 CFR 1910.119(f)(4):The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: The employer failed to implement the ammonia line break procedure for the replacement of barrel tubes for Thermutator's 1, 2, 3, and 4 in the Taco room located at 512 S. Main Street, Hutchinson, Kansas. The employee(s) were in the process of removing the old barrel tube from Thermutator 1 when an unexpectant release of ammonia gas engulfed the room exposing employee(s) to burn / inhalation hazards.
