29 CFR 1910.178(a)(4): Modification or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: At the workplace located at 10221 NW Transcon Drive, Kansas City, Missouri: Employees were exposed to struck by and tipping hazards for the Hyster # 6 forklift. The Cascade push pull attachment was added without the manufacturer's prior written approval. The Cascade push pull attachment was added to the Hyster # 6 forklift and the serial plate with the capacity had not been updated to reflect the weight of the Cascade push pull attachment.
