29 CFR 1910.176(b): Storage of material created a hazard: (a) Facility, 108 Grundman Drive, Butler, PA: On or about November 10, 2020 tempered glass panels being stored on an a-frame cart created a hazard in that the cart did not have locking wheels and chocks were not used to ensure the cart did not move while employees placed or retrieved panels. An a-frame cart shifted when employees were tilting panels to obtain a certain piece. The shifting of the cart caused the glass panels to fall over striking an employee.
