29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years: The employer is failing to ensure that employee(s) who operate powered industrial trucks were evaluated at least once every three years. This violation occurred on or about November 10, 2020 and at times prior thereto where employees were exposed to struck-by hazards when employees were required to operate powered industrial trucks while moving material without having been evaluated once every three years.
