29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded: a) production floor, on or about November 3, 2020, the point of operation on the Cincinnati hydraulic press brake was inadequately guarded. The Triad Superlight VI light curtain was not operating properly to protect operators from the pinch-point created by the hydraulic press during normal operations. On or about November 3, 2021, an employee experienced amputation injuries to the right hand while operating the press when the light curtain/controller system failed.
