29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. On or about October 28, 2020, an employee was exposed to a cave in hazard while performing waterline repair at 5446 Hollywood, Maple Heights, Ohio 44137. The employee was working in a trench that was approximately 5.5 feet deep without cave in protection and the employer did not ensure that the accumulated water was removed from the bottom.
