1910.147(a)(2)(ii) Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as minor tool changes and adjustments, and other minor servicing activities. The employer did not ensure that the alternative measures that were provided to the employees performing minor servicing and maintenance activities - cleaning operations on a Harris M10000A Offset Printer Equipment Number N 212 - during normal production operations were sufficiently effective.
