Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling objects due to improper rigging: a. Site - On October 7, 2020 an employee was killed when he was fatally struck in the head by a 50' Flextral AR60-400 Wire reinforced 4-inch air hose which was equipped with a steel fitting and a protective sheath that had broken free from a winch located at the top of the mast of a SoilMec SR-30 drilling rig. The hose was hoisted by a Dixon King Cable hose-to-hose service safety cable, a wire rope that is designed to be used to secure connections between compressed air lines so in the event that the connection is broken the hose does not turn into a projectile. This cable is not designed as hoisting equipment and as such, the wire rope contained no markings to indicate the rated capacity as typical hoisting equipment would have been. The wire broke under the weight of the hose and sent the hose falling approximately 40 feet to the ground below where it struck the employee.
