Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) On or about October 7, 2020 and at times prior to that date, for the employees exposed to impalement hazards from protruding metal form stakes while pouring and finishing concrete located at the Beale Security Gate Repair Project Nellis AFB, Nevada. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to remove or provide a protective covers over the metal form stakes.
