Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that; employees were exposed to struck-by and crushing hazards: On or about October 1, 2020, and at times prior, pieces of wood were used to push the contents from Vestil D style-dumping hoppers, that were elevated on the forks of powered industrial trucks, because the forklifts were lacking serviceable security pull cables, thereby; exposing employees to struck-by, crushing, and amputation hazards. Recommended Abatement Action: Among other methods, feasible means of abatement would be to ensure all self dumping hoppers are properly secured to powered industrial trucks; all safety devices on self dumping hoppers maintained in serviceable conditions; and operators for this type of equipment trained on its proper use.
