OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious bodily injury and death while riding on the rear step while the waste collection vehicle was operating in reverse: (a) Jobsite, Railroad & Main Streets, Portage, PA: On or about September 22, 2020 an employee was riding on a rear step of a waste collection vehicle while the vehicle was backing into an alley at a weekly scheduled stop. The vehicle struck a utility pole crushing the employee between the vehicle and pole resulting in fatal injuries. The employer did not ensure that employees were not riding on the rear side steps while the vehicle was operating in reverse
