29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Slitting Room: On, or about, September 11, 2020, adequate machine safeguarding had not been provided in the following slitting machines to protect operators from the following hazards: A. Slitter 5: Pinch point where wrapper paper was inserted between rotating shafts that created a pinch point. B. Slitter 1, Dusenbery: Pinch point where wrapper paper was inserted between rotating shafts, and the in-running slitter knives, including the meshing gears located at the end of the knife shaft. C. Slitter 2: Pinch point where wrapper paper was inserted between rotating shafts, and the in-running slitter knives, including the meshing gears located at the end of the knife shaft. D. Slitter 4: Employees were exposed to rotating parts while setting up and operating the machine. Although emergency stop control buttons were provided, a trip-wire was needed to protect the entire operator�s work platform.
