29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 09/02/2020 in the HTB department; where an employee was injured while lubricating the HTB dryer shaft bearing and checking the tension on the drive belt. The equipment was running and the employee was exposed to a belt/pulley hazard. The equipment was not "shut down and locked out". The employee's finger was amputated in the belt/pulley pinch point. ABATEMENT DOCUMENTATION REQUIRED
