29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Brake Department: On or about August 31, 2020, and at times prior, the employer exposed employees to amputation hazards, in that employees operating the Accurpress Model 71550 press brake to form metals were not protected from the point of operation by a guard designed and constructed to prevent the operator from having any part of his body in the danger zone.
