Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about August 20, 2020, located at 3900 Bixby Road, Groveport, Ohio: the employer did not develop a documented energy control procedure (lockout/tagout, LOTO) for, but not limited to the FR-100 stencil machine, 2.0 Coding and Marking System, Rea Jet print head. The system was not de-energized prior to the troubleshooting/commissioning, which resulted in the fingertip amputation of an employee who conducted the task, when his finger was caught-in between the mount and guide rail.
