29 CFR 1910.147(c)(4)(ii)(B): The employer did not establish specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: Or in the alternative: Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: (a) On or about August 20, 2020, for the employee who suffered an amputation of fingers on the left hand while attempting to clear a plugged auger on a feed truck, at or near 157th Avenue in Rhame, North Dakota.
