29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: 1-6 Takeaway Lane - On or about August 18, 2020, an employee was seriously injured while performing conveyor maintenance. A lockout device was not affixed to the conveyor motor to prevent the unexpected energization of the conveyor belt.
