29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: a) Care One at Livingston Assisted Living, 76 Passaic Avenue, Livingston, NJ 07039 The employer failed to record the death of a Certified Home Health Aide who died of a confirmed case of COVID-19 on April 8, 2020. The employer became aware of the fatality of April 9, 2020. Violation occurred on or about April 9, 2020.
