Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of struck by and contact with carbon dioxide gas, an asphyxiant in high concentrations, from the operation of a process involving the filling of 100 pound cylinders with carbon dioxide from a bulk tank the process piping of which was not tested at sufficient pressure to ensure adequate design and construction free from defect after its initial construction and after a subsequent repair: (a) Location - 400 Main Street, Ashland, MA: On or about 08/18/2020, a process for filling 100 pound cylinders with carbon dioxide from a bulk tank was operated by employees. The piping responsible for transferring the carbon dioxide from the bulk tank to the cylinder filling stations in this process has never been tested to ensure it can withstand the pressures and other forces imposed upon it during this process.
