29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: a) John W. Kachurak, DMD, PC - On or about August 20, 2020, employees performed dental work on patients and processed contaminated dental tools and the employer did not make available the hepatitis B vaccine, nor had recorded evidence of vaccination status or a declination form for declining the hepatitis B vaccine. John W. Kachurak, DMD, PC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(f)(1)(i): , which was contained in OSHA inspection number 1436621, citation number 1, item number 2 and was affirmed as a final order on February 28, 2020, with respect to a workplace located at 522 Juniper St. Quakertown Pa 18951 Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
