29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) On or about 4/23/2020 at 4915 10th Avenue Brooklyn, NY 11219, a certified nursing assistant died on 4/17/20 as a result of complications from a COVID-19 infection. The employer did not record the death on the OSHA 300 Log. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY.
