OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/caught between hazards from not utilizing a powered industrial truck operator protection device or system: a) Manufacturing Warehouse - On or about August 14, 2020 employees operating a Hyster powered industrial truck (Model # H50FT Serial # L177V06013H) were not utilizing the truck's seatbelt, exposing them to struck-by and crushed by hazards.
