The employer did not ensure that no employee uses any anchorage before the employer has obtained written information from the building owner that each anchorage meets the requirements of paragraph (b)(1)(i) of this section. The employer did not keep the information for the duration of the job: On or about August 14, 2020, written documentation to show that each anchorage point affixed to the roof of the building was identified, tested, maintained, and certified that it was capable of supporting 5,000 lbs., was not obtained from the building owner prior to the start of any/all work activity. Employees who used seat board when completing window-washing activities were potentially exposed to falls of approximately 96 feet, from the top of the roof to ground level.
