29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about July 28, 2020 at the worksite located at 7167 Washington St. SW Covington, Georgia an employee utilizing a disc sander received an amputation while the employee was attempting to deburr a piece of metal. The employer failed to notify the agency within the prescribed time. The agency was made aware of the amputation on or about August 5, 2020.
