29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a) On or about August 5, 2020, and at times prior, Total Welding, Inc. did not ensure that lifelines were protected against being cut or abraded. A vertical lifeline used by Total Welding, Inc. employees was set up using a 3/8" galvanized steel cable wrapped around 90 degree corners over the top of a concrete masonry unit wall without any protection against cuts or abrasion exposing workers to fall hazards greater than 15 feet.
