29 CFR 1910.212(a)(3)(ii): The point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. a) Worksite, machine shop, press brake #4: On or about August 3, 2020, an employee was exposed to amputations while operating an Amada HFBO Press Brake (S/N 100 30 R981122) that did not have a point of operation guard. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
