The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and struck-by hazards from a moving elevator. At the Bayside Center 1, by the two 4 stop elevators: On or about July 31, 2020, employees working in the middle of the elevator shaft between Elevators A and B, while Elevator B was operational, exposed to crushing and struck-by hazards.
