Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Multiples Dept., Wickman 1 3/4" C 6-Spindle Automatic Screw Machine, equipped with Excelda No Cam Pickoff Attachment. On or about July 20, 2020, operators who performed servicing and maintenance tasks, including cleaning of the pickoff attachment collet as well as setup tasks, were exposed to amputation and crushing hazards to hands/fingers from the unexpected startup/movement of the attachment components. Employees were able to operate control switches for the pickoff attachment with one hand at the same time the other hand was inside the machining area. Employees were not required to deenergize and lockout the machine's electrical and pneumatic energy sources prior to entering the danger zone with their hands to perform servicing and maintenance. Employees did not control hazardous energy sources in the following sequence: 1) turn off and shut down the machines, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy, 5) verify that isolation and deenergization of the machines or equipment have been accomplished. Alternative protections, such as machine guarding, were not adequate. Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).
