29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. On or about July 23, 2020, in the facility, employees were exposed to hazards including, but not limited to electrical and laceration hazards when the energy control procedure for Combi Model 2EZHS Ergopack Serial Number 2EZHS588660 on Line 19 omitted a source of energy. The omission of the energy source exposed employees to unexpected start up of the machine.
