29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout and opening procedures. These safe work practices were not applied to employees and contractor employees: a) Leprino Foods Company, 1302 1st Ave, Greeley, CO 80631: On and before June 22, 2020, the controlling and exposing employer did not develop and implement safe work practices such as lockout/tagout, de-energization, and line opening procedures to protect employees and contract employees from hazards of pneumatic pressure testing when two new 12-inch-diameter chilled water piping, located in close proximity to existing anhydrous ammonia process piping (e.g. within 11 feet of 1.5 inch-diameter anhydrous piping) were left pressurized and unprotected, exposing Leprino employees, Refrigeration Concepts Inc. (RCI) and Ulrich Process Piping contractors to pressure blast, fragmentation (struck by), and ammonia hazards. Note: For requirements regarding the process safety management of highly hazardous chemicals as it pertains to construction work per 29 CFR 1926.64, follow the requirements in 29 CFR 1910.119.
