29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) On or about June 19, 2020, the employer failed to ensure that employees exposed to a pinch point when loading a metal piece of tubing into the automatic hydraulic clamps of the Bentec BT-220 CNC tube bender, machine number PKM-0721, were protected by adequate machine guarding, leading to an amputation. b.) On or about June 30, 2020, the employer failed to ensure that employees exposed to a pinch point when loading a metal piece of tubing into the automatic hydraulic clamps of the Bentec BT-220 CNC tube bender, machine number PKM-501, were protected by adequate machine guarding.
