29 CFR 1910.269(o)(3)(ii)[C]: For employees working transmission and distribution lines or equipment as deenergized, the employer did not ensure that the lines or equipment were deenergized under the provisions of paragraph (m) of this section and did not ensure proper grounding of the lines or equipment as specified in paragraphs (n)(3) through (n)(8) of this section: a) On or about June 23, 2020, at Power Station 3, 35 Gen substation of Motiva refinery, all breakers and switches were not grounded when employees were working on them as deenergized.
