29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact. (a) Facility, Motor 256 (Tote Bag U-Through) on or about June 17,2020, employee(s) were exposed to moving parts (i.e. flow valve) inside the grain chute, due to grain chute components not installed prior to machine startup.
