29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: The employer failed to ensure that employees who completed service and/or maintenance activities such as, but not limited to, the following were protected from fall hazards: a) Production area: On or about 05/05/2020, an employee in the process of troubleshooting the inlet pipe of the holding tank on Line three (3) was allowed to climb on top of the unit. The employee was exposed to a fall hazard of approximately eight feet and seven inches, from the top of the tank to ground level. b) Production area: On or about 06/08/2020, an employees working in the process of service and/cleaning debris from the Baker Perkins Direct Fired Tunnel Oven Model #960 was allowed to climb on top of the oven(s) with no fall protection and/or railing. The employee was exposed to a fall hazard of approximately five feet, eight inches from the oven to ground level.
