29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: On or about June 11, 2020, the employer failed to ensure that adequate precautions were taken to prevent the ignition of flammable vapors. Employees used a propane torch to attempt to open a chemical tote which had held flammable liquids with a flashpoint of 266 degree F.
