29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) On or about 06/18/2020, in the production area; Lockout procedures were not utilized when a press operator was clearing scrap metal from under the ram of the 100 ton Niagara Press. He accidentally actuated the press when his left hand was under the ram resulting on four (4) partial finger amputations. ABATEMENT DOCUMENTATION REQUIRED
