On or about 6/12/2020 at 4663 East Lake Road, Williamson, NY, An employee was working in close proximity to the bucket of a John Deere 240D excavator, for the purpose of placing rock onto a seawall and ensuring that the structural fabric remained in place while the rock was being set, a) The worker was not provided the appropriate high visibility safety apparel. b) The operator did not follow correct procedures when he lost sight of the worker on the ground and continued to operate the bucket. c) Prearranged hand signals to communicate with the ground person were not established prior to work. Abatement documentation must be submitted for this item.
