29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about 6/11/20, at the site of Buffalo, NY. Employees did not wear personal protective equipment such as: flame resistant clothing, during the process of adding water into a reactor kettle of heated oil and melted stearic acid. Once water was added, a strong reaction produced the heated oil and melted stearic acid solution to come out of reactor kettle burning employees on their back, arms, and legs. ABATEMENT DOCUMENTATION REQUIRED
