29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: (a) At the worksite: On or prior to 6/6/20, while attempting to remove slabbed paper and rethread the PM-11 rewinder, an employee had his arm caught in and crushed in an in-running nip point. The employee did not receive treatment for almost 20 minutes due to procedural delays in contacting off-site emergency medical personnel.
