Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.269(a)(4): The employer failed to determine the existing characteristics and conditions of electric lines and equipment that are related to the safety of the work to be performed before work on or near the lines or equipment is started. Such characteristics and conditions include, but are not limited to: (i) The nominal voltages of lines and equipment, (ii) The maximum switching-transient voltages, (iii) The presence of hazardous induced voltages, (iv) The presence of protective grounds and equipment grounding conductors, (v) The locations of circuits and equipment, including electric supply lines, communication lines, and fire-protective signaling circuits, (vi) The condition of protective grounds and equipment grounding conductors, (vii) The condition of poles, and (viii) Environmental conditions relating to safety: (a) On or about June 1, 2020, FieldCore Service Solutions, LLC, did not determine the existing characteristics and conditions of the electrical lines and equipment in that guards were not provided around live parts operating at more than 150 volts to ground without an insulating covering to minimize the possibility of accidental employee contact. A FieldCore Service Solutions, LLC, worker was exposed to electrical hazards while working with live parts operating at 330 volts due to stored, unused, energized cables at the top of wind turbine generator #13. There were no guards around all live parts. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
