29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: a) Barber Foods, located at 54 St. John Street, Portland, Maine - On or about 05/13/2020, an employee tested positive for COVID-19 and was away from work for 28 days. The employer did not record the work related illness on the OSHA 300 log or its equivalent. b)Barber Foods, located at 54 St. John Street, Portland, Maine - On or about 05/13/2020, an employee tested positive for COVID-19 and was away from work for 17 days. The employer did not record the work related illness on the OSHA 300 log or its equivalent. c) Barber Foods, located at 54 St. John Street, Portland, Maine - On or about 05/14/2020, an employee tested positive for COVID-19 and was away from work for 13 days. The employer did not record the work related illness on the OSHA 300 log or its equivalent. d) Barber Foods, located at 54 St. John Street, Portland, Maine - On or about 05/18/2020, an employee tested positive for COVID-19 and was away from work for 11 days. The employer did not record the work related illness on the OSHA 300 log or its equivalent.
