29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: On or about May 20, 2020, on the loading end of SMETCO #2, employees were exposed to electrical hazards while working near an electrical junction box that had conductor wires entering the box without the box being effectively closed.
