29 CFR 1910.212(a)(3)(ii): The point of operation of machinery was not guarded to prevent employee(s) from having any part of their body in the danger zone during operating cycles: On or about May 6, 2020, the employer did not ensure that the point of operation of the PEXTO metal shear was adequately guarded to prevent employees from accessing the hazard area at the time of operation. An employee suffered an amputation injury after the shear blade came down on their left middle finger.
